Outreach Project

We provide specialist, person-centred, and outcome-focused outreach support for adults aged 18 and over with disabilities, living in the Sandwell area. Our goal is to promote independence and enable individuals to live as independently as possible within their communities.

Our Outreach service is tailored to your strengths, needs, and personal goals. Through a strengths-based assessment and a person-centred approach, we offer the right level of support to help you access your community, take part in meaningful activities, and develop new skills.

We support people in working towards both short- and long-term goals, whether that’s building confidence, learning life skills, or exploring volunteering and future opportunities.

From trying new things and staying active to connecting with others, our Outreach programme is designed to empower you through engaging, inclusive experiences. Support is always based on your interests—whether it’s going to the gym, attending a volunteering placement, or enjoying a game of bowling with friends.

Our experienced staff work closely with individuals and their families to create personalised support plans. These outline your needs, goals, and preferences, ensuring we provide the most effective and meaningful support.

We focus on helping you maintain independence and, where needed, develop new independent living skills. Outreach sessions typically last 3 hours and, where appropriate, individuals may be paired with others based on shared interests and location.
