Safer Sandwell Partnership Local Police and Crime Board published a domestic homicide review on 4 June 2025.
In line with Home Office guidance, the Partnership (Sandwell’s Community Safety Partnership) commissions and publishes independent reviews into deaths of residents aged 16 or over which have resulted from violence, abuse or neglect by a relation, partner or member of the same household.
This review followed Angela* being killed in 2022 by her husband Miles*, who was subsequently convicted of her murder and jailed.
Kim Madill, Chair of the Safer Sandwell Partnership, said: “While incidents like this are thankfully rare, they are absolutely devastating. My thoughts and condolences are with everyone affected by this very sad case.
“We commissioned this independent review to identify any areas of learning for partnership organisations.
“Although the review found there was very little relevant agency contact in this case, it has still highlighted areas of learning and opportunities for strengthening our multi-agency response to domestic abuse.
“Organisations are implementing the review’s recommendations. This has included updating and increasing training for professionals and reviewing how domestic abuse is documented in forms and processes.
“We must do all we can to ensure professionals ‘ask the question’ about domestic abuse and offer the right support at the right time. The review emphasises the need to make every contact count. Victims need to have complete confidence to approach services that can offer them potentially life-saving support.
“The Domestic Abuse Act 2021 has also put in place wide-ranging measures to protect people experiencing domestic abuse and challenge behaviour of those responsible for abuse.
“On behalf of Safer Sandwell Partnership, I would like to thank all the professionals and agencies who have contributed to this review, to help strengthen the way we tackle domestic abuse and provide support.”

People can also seek help from their GP if they are experiencing domestic abuse.
Domestic abuse cases are given priority in courts and are always heard before a trained bench and staff who are trained in this field. Specialist third party services are available at courts to support victims and witnesses.
