Our approach

We begin planning for young people who are likely to meet the eligibility criteria under the Care Act 2014 around the age of 14. 

The Care Act sets out the legal responsibilities for adult social care, so it's really important to us to start planning early for when a young person turns 18 years.

We will: 

  • carry out an assessment that covers all parts of the young person’s life and support needs. 
  • work closely with the young person and their family to prepare ready for adult life
  • provide information, advice and support to understand information and express their needs and preferneces.
  • facilitate the move to adult social care services at the right time for the individual.
