Our 12 week pathway provides support for 12 weeks for adults with learning disabilities and/ or autism.
The aim of the pathway is to provide opportunities for people to build confidence and grow their independence whilst being part of the local community.
The 12 week pathway team work with people to plan and develop their future - this can be finding suitable services, or wider opportunities in the community.
