What Is an EHCP?
An Education, Health and Care Plan (EHCP) is a legal document for children and young people aged 0–25 who need extra support due to SEND.
It sets out:
• The young person’s needs
• The support they require
• The outcomes they want to achieve
• Long‑term goals for education, health, care and adulthood
An EHCP ensures coordinated support across education, health and social care.
How we may support the EHCP Process
Personalised Careers Guidance
We may provide impartial, one‑to‑one advice to help young people explore their strengths, interests and ambitions. This contributes to the Preparing for Adulthood section of the EHCP.
Attending EHCP reviews
Our advisers may attend annual reviews to:
• Share careers insights
• Discuss realistic and ambitious options
• Ensure the plan reflects the young person’s goals
Supporting transition planning
We can help young people prepare for their next steps, including:
• College courses
• Supported internships
• Apprenticeships
• Traineeships
• Employment pathways
• Community and independence‑building opportunities
Working in Partnership
We collaborate with schools, SEND teams, health professionals and families to ensure careers guidance is joined‑up and consistent.
Keeping Aspirations at the Centre
We, along with partners, aim to help young people express their goals, so their EHCP reflects their voice and long‑term ambitions.
If You Are Not Happy With an EHCP
You can appeal parts of the plan to the SEND Tribunal, including:
• Part B – description of needs
• Part F – provision required
• Part I – named school or setting
Sandwell SENDIASS provides independent advice and support to help parents and carers involved in discussions about their child's SEN and can advise parents on the EHC application process and help with concerns if your child has an EHC plan
