Who is the service for?Some of our services targeted specifically at African Caribbean & Duel Heritage Communities (Adults/Older People & Carers) who either have long terms health conditions, continuing care needs or that address loneliness and isolation. Our Self Care/Prevention and Care Navigation Services aim to address health inequalities by accessing support services that add years to life and improve health outcomes for all citizens and communities who live and work in Sandwell.
