| Summary of services | • Emergency respite care and support • Advocacy support Information and Advice, and training in carers own homes (with back fill care for cared for person) on -Moving, Handling and Hoist Training • First Aid training • Catheter and Stoma Care training • Moving, Handling and Hoist Training • Dementia Care Assistance with Shopping and meal preparation Bereavement support, before, during and after a death. An initial assessment to help identify agreed goals, support is given through regular face to face meetings. Each Carer will receive person centred support based on their individual needs. Invite carers who have received support to join the team as ‘support enablers’ |
