Summary of services | Supported living provides an opportunity for individuals who are unable to live completely independently, to live in their own home assisted by dedicated support staff. This style of living is provided in over 35 well-equipped and individualised properties ranging from 1 bedroom flats to 4 bedroom bungalows. These properties are either ordinary houses in ordinary streets or form part of small complexes designed to reduce social isolation. Each property is selected based on the needs and choices of the individual/s who are going to live there. Each person living in supported living has an individual care plan outlining the exact level of support to be provided. This level of support is aimed at enhancing and maintaining an individual's independence whilst acknowledging and addressing any personal risks. The levels of support provided can be as little as 2 hours per day for very independent individuals up to 24 hour support for those with profound disabilities and complex needs. Support is provided by trained, experienced and committed staff that are specifically chosen to work with individual service users (service users having the final say in who their support workers are). This allows a personal as well as professional relationship to develop between the service user, their families and the support worker/s. |
