Komplex Care provides services primarily within clients’ own homes and communities across the UK. Our care is delivered in individuals’ homes, supported living environments, schools, workplaces, and community settings, depending on the client's needs. We also work closely with healthcare professionals, case managers, hospitals, and multidisciplinary teams to ensure coordinated care.

Who is the service for?
Children, young people, adults and older people of all genders with complex health needs, disabilities, long-term conditions, or injuries. Services are provided to individuals living in their own homes and community settings, including throughout the Borough of Sandwell.
Referral route
Referrals are received from local authorities, NHS services, hospitals, case managers, social workers, healthcare professionals, commissioners, and private individuals/families. Direct self-referrals are also accepted.

"Komplex Care is a specialist nurse-led care provider delivering high-quality, person-centred care and support to children, young people, adults, and older people with complex health needs, disabilities, acquired brain injuries, spinal injuries, neurological conditions, learning disabilities, and long-term medical conditions. Services are provided in individuals' homes and community settings, enabling people to live as independently, safely, and comfortably as possible. Our mission is to empower individuals to achieve the best possible quality of life through compassionate, tailored care that promotes independence, dignity, choice, and inclusion. We work collaboratively with individuals, families, healthcare professionals, local authorities, NHS services, and case managers to deliver safe, effective, and outcome-focused care that meets each person's unique needs. When are the services run?
Services operate 24 hours a day, 7 days a week, 365 days a year. Care packages are tailored to individual needs and may include daytime support, overnight care, waking nights, live-in care, and emergency or complex care provision.
