People over the age of 18 who live in Sandwell and who have been discharged from hospital and need help settling back into a normal routine.

They provide short-term care and support in the home for people after an accident or illness, giving them the confidence to continue their daily lives. This service can be provided following a stay in hospital or to prevent unnecessary hospital admissions.The support offered by their volunteers can smooth the process of settling back into a normal routine and enable people to regain their confidence and independence. The service includes:rebuilding confidence collecting prescriptions offering companionship assistance with shopping.The service does not include personal care or domestic duties.The service is available on a time-limited basis for up to 6 weeks.
