| Who is the service for? | STAR is a re-enablement and assessment service for adults that are leaving hospital, rehab/intermediate care beds or the Fast Response service, or people within their own homes who have not had a care package previously or need a significant increase in their care package. Any adult who needs support to return home/or stay at home to maximise their independence and carry out a full assessment of needs in their own home. |
