Assessment of health and social care needs

A health and social care assessment is often the first step towards getting the help and support you need.

You can contact Sandwell Council ASSIST, or speak to your doctor, district nurse, or another health or social care professional. If you are in hospital you can talk to staff on the ward.

If you call Sandwell Council ASSIST we will ask you a series of questions to find out what sort of help you need.

You may just want information or for us to direct you to another service.

If you need more support, we will talk to you over the phone to find out exactly what your needs are. We will ask you a series of questions about your circumstances and the help you feel you need. This is called a self-directed assessment. The law says that we have to assess anyone who may need a service from us.

Support for carers

Your carer also has the right to their own assessment. A carer is someone who regularly provides care and support for a relative or friend. You can find our more about support for carers in our carers pages.

We aim to help vulnerable people who live in the borough to keep their independence, stay safe and enjoy a good quality of life.
