Domestic Homicide Review - Published 8 October 2015

Safer Sandwell Partnership logoThe Safer Sandwell Partnership published a domestic homicide review on Thursday 8 October 2015.

The Home Office asks us to publish independent reviews into deaths of adults which have resulted from violence, abuse or neglect by a relation, partner or member of the same household.


Chief Superintendent Matt Ward, chair of the Safer Sandwell Partnership, said: “This is a tragic case where a man killed his wife and then himself.

“Our thoughts and condolences are with their family as we publish this review.

“In line with Home Office domestic homicide review guidance, we commissioned an independent review to identify what lessons could be learned from the case.

“The report says there is nothing to suggest partnership organisations could have predicted or prevented this tragedy. Nor did they miss any opportunities to intervene.

“While there are no lessons or recommendations coming out of this particular review, we continue to work together as agencies to do all we can to prevent domestic violence.”
